It is you who is responsible for the task (nonstandard).
It is you who are responsible for the task (standard).
If you cannot say “you is”, then you cannot say “it is you who is”.
It is I who is to prepare the manual (nonstandard).
It is who am to prepare the manual (standard).
If you can’t say “I is”, then you can’t say “it is I who is”.
